Cannot run the FrontPage Server extensions error problem

I have a web-site posted at www.swsurfers.org and go click the UBS tab and you get right to my site. Once you enter your name and phone and hit submit it works great - taking you to my main page - the problem is when you click on "schedule a free consultation", "tell a Friend", and "I need more information" it takes you to the next page fine, but when you fill in information on these pages and hit "submit" - I get this error msg: "Cannot run the FrontPage Server Extensions on this page: "http://www.swsurfers.org/My_UBS_project/Schedule_an_Appointment.htm" - What should happen is - it should send me an e-mail of the results and go to the next page of the site which I have placed in the confirmation page Any suggestions? Thanks in advance for your help.

How did you create the first form as compared to the other that don't work,
how were they uploaded to the server, etc.?

Thomas - I created the front page (that asks for name and phone) and put the main page in the "confirmation page" under form properties - that's the sam thing I did on the pages that do not work. In other words when you hit submit on the front page (that asks for name and phone) then the FP sends me the results by e-mail and then send you to the main page because it is told to go to the confirmation page with the main pgae address - this is the same thing I did on the other pages. Any ideas? Do you think it has to do with how it is uploaded

You must be able to either create the form directly on the site in FP or
upload to the site via FP's http mode.
