Can't find Field List Icon on the toolbar

[SFC Lamp]

I am creating a multi-page form and need to create a tab control within it.
For my assignment, I have to input data using the Field List button which I
can't find on the toolbar.

Please help

It's under the View menu, not the toolbar.
