Can't fix incorrect paragraph numbering

My document has sections w/ headings as follows:
3.3
3.3.1
3.3.2
3.3.3
3.4
3.5
3.1.1 <<< I cannot get this "fixed" to show 3.5.1 like it should.
3.1.2 etc

I've tried everything I can think of and get nothing but wrong results:
When I chose Format>Bullets & Numbering, it shows "Continue previous list" alread checked.
Turning numbering off then back on for that heading changes it to 3.6.
I've tried to customize the numbering, but only get 3.1.2, 3.1.3 etc instead of 3.5.1, 3.5.2 like it should.

FWIW, the TOC picks up this error and shows the same incorrect numbering.

I've had this happen in Word 97, and now Word XP. What am I not doing properly??
