Can't remove "Visual Basic" toolbar & cannot add toolbars thru cus

Hi,

I Can't remove the "Visual Basic" toolbar which has positioned itself right
in the centre of the screen; it can't be removed or moved

I also cannot add toolbars through the customise menu.
