can't view switchboard form in design view

I created a switchboard using switchboard manager and
can't view it in the list of forms available, so I can't
set it as the default form on startup either. It is there
when I go into switchboard manager but I can't see it in
the forms list.

Any advice gratefully received.

It sounds like the switchboard form itself was never
created. Erase the table called SwitchboardItems and re-
create the switchboard with switchboard manager again.

It would probably work better to do the following:

1. Rename the SwitchboardItems table to SwitchboardItems_1
2. Run the Switchboard Manager to create a new switchboard.
3. Delete the new SwitchboardItems table.
4. Rename the SwitchboardItems_1 table to SwitchboardItems.
