Captions skipping numbers

I have applied everything I have read about using Field
Codes to number captions (with the chapter number) so that
I display "Figure 2-1" for example.
But Word is skipping numbers in one particular document
that I inherited.
It goes from 2-1 to 2-3, for example. I've examined the
field codes and they seem correct. The first has the SEQ
Figure \r 1 configuration, the next is SEQ Figure \n.

The chapter number is picked up from Heading 1 in the
field code STYLEREF 1 \s. Then there's a hyphen, then the
Figure field.

Any ideas?

Are you tracking changes? Note also that you do not have to create these
fields by hand; if you use the Insert | Caption dialog (selecting "Include
chapter number"), this will be handled for you automatically.

No, not tracking changes. And the captions WERE created
using Insert Caption. I'm trying to fix them by examining
and modifying the field codes.

Could some of the fields have been formatted as hidden? If this is the
case, clear the hidden formatting.
