CD Stomper label

Hi All, I am looking for a Word template to make CD labels on cd stomper paper. The Avery paper doesn't match, wrong configeration.

The CD Stomper template normally comes with the Stomper kit; do they have a
web site? if so you may be able to down-load it from there.
