How would I go about creating creating a question on the form that can
consist of more than one answer (a "check all that apply" type of question)?
Would I create different fields for each option and make them yes/no? Or is
there a feature on the option box that allows me to have more than one
answer? Any suggestions would be great! Thanks!

The correct method is to have a record for each answer. That is, use a
separate table with a subform to record each answer.
