Check box in email required - how?

I want to send out an email with a check box so that when the recipient
replies, they can optionally tick the box to state they agree to the text in
the adjacent paragraph (for example).
I realise that not everybody has Outlook. I've tried creating an HTML
Outlook template file. Help!
I don't want to refer the recipient to a web page due to the additional cost
of creating one.

But even if you succeed, how will you detect when they click the check box?

Hi Brian

I will know when the check box is ticked because when the recipient replies,
Outlook includes my outgoing message - and the tick box. Surely if my text
requests the recipient to "tick the box below" when replying, I should see
the box ticked when I receive their reply.

Is it not possible to edit an HTML Outlook template or .msg file so that I
can drop a check box into it?

IMHO, you are over-engineering the problem. I just do this:

[.] Type an "X" in the box if you agree.

[X] (Implied: I agree.)
