Check box, not yes/no

Grrr.
I don't WANT yes/no... I want a check box with a check indicating YES. I'd
like this in my original table & in all queries or reports deriving
therefrom. Can't seem to figure it out... and the maddening thing is that
I've done it successfully in the past.

Help??

Table design view. Select field and look at the Lookup tab at the bottom. Set
the control type to "CheckBox".
