Checkbox in forms stop working

I created a form and tested it out and it worked fine, but after I
saved it and tried to use it, the checkboxes would not work. It only
opened Visual Basic - which I have no idea how to use, by the way.
FYI - The form is protected and I used the checkboxes from the forms
toolbar and textboxes from both forms toolbar and control toolbox.
Why won't the checkboxes work after reopening?

What are you using a mix ("from both forms toolbar and control toolbox")?
If your document is protected for forms, you should use only formfields.
