Checkbox?

I want to create a publisher document (2003) with checkboxes for people to
fill out as an information card. I don't think I want to convert the doc to
a web publication, because I am unable to view print layout accurately. I
don't know of any other way to add check boxes except with the forms toolbar.
Inserting a symbol does not give me the correct box size. What are my
options?

Publisher is not designed to create interactive publications.

Is this intended as a printed form or an interactive form?

If the former, the Wingdings font has some nice checkbox symbols.

If the latter, you'll need to convert to Adobe PDF and add form fields, or
use a web publication.
