Checkboxes print incorrectly

Hi all
I have a word doc with 10 checkboxes. I check some and
all looks fine in print preview. When I print other
checkboxes appear checked.
Thanks for any help offered.

Which version of Word?

How, exactly, were these checkboxes inserted into the
document?
