Collect information from a Word form into a table

Hello,
We'd like to gather information to place in a table via a form. We would
send and receive the form via e-mail.

Can anyone give me general instructions as to how this would be done? I
assume designing the form as a template in Word. I'm not sure how then the
information would be imported into Access.

Thanks, Arvin. Yes. Excel and Access interfaces quite well. I'll
investigate making some sort of a form in Excel.

I'm not sure as to how your script works, however.
