Combine names and address in a textbox

I need help on how to combine name fields and address in a texbox as follow:

John Doe
1234 Anytown Street
Any City, AnyState 00000-0000

Your suggestions will be highly appreciated!

I'd use three text boxes....

=[FirstName] & " " & [LastName]
=[Address]
=[City] & ", " & [State] & " " & [ZipCode]
