Commercial Printing problems

We are encountering the following problems with Publisher when we go to print
to our rip.
1. Our customer designed an 11 x 8.5 three fold brochure on an 11 x 8.5 page
in publisher. It has bleeds. In order to print with bleeds we need to make
our page bigger than 11 x 8.5. When we select any page larger than the page
the document is set up on it reflows everything and even changes layering in
graphics, bringing them forward on top of type.
2. Black type prints out in four color process instead of just black.

One last question, in the past I have read that if you print to a postscript
file out of publisher or even to a pdf that even if you have selected CMYK
for printing it changes back to RGB. Is this still true?
