Complete Flags still show-up in To-Do Bar

I am using Outlook 2007. When completing some (not all) of my flags. The
item remains in the To-do Bar. Other times the item disappears. I would
expect and want all completed items removed from the To-do bar. Any ideas
what might be wrong?
