Completed Tasks Re-Appear as Incomplete

Hello,
We are having an issue with completed tasks re-appearing as incompleted
tasks. The tasks are marked as 100% complete, submitted to project server and
then accepted by the respective Project Manager. The schedule is then
republished. The tasks then re-appear in the task owners to-do list as
incomplete. We have tried submitting this several ways with no improvement.

Has anyone experienced this? Any ideas?
