Compliance Form

I want to set-up a datasheet form that lists employees down the left side of
the datasheet and the days of the given week or date range on the top of the
datasheet. The form will allow an input indicator "X" to be entered in the
datasheet by employee and day if the employee met the requirement for that
day. I want the datasheet to automatically display the dates across the top
for a given week or date range. The compliance information would be entered
once a week for the previous week or weeks.

What would be the best way to set up this type of form?
