Contact name & Business name in contact title

In Outlook 2000 how do I get the person's name and their business name to
appear in the tan colored box at the top of the contact for viewing &
printing? The cards are in the "Detailed Address Cards" view. I set this up
once before, however my computer crashed and I can't remember how I did it.
