contact selector problem

i created a form that uses a "contact selector" to allow users to select the
person making the request.
The form is submitted by email. When a email address is selected using the
"contact selector" instead of showing the name the field grays out. Then when
the form is submitted all the fileds show up as they should except the
"contact selector" field which shows up as a black rectangle.
Any help would be greatly appreciated.

thanks for the tip however as far as i can tell after going through this
several times and creating several test forms everything is configured as it
is supposed to be. yet the problem remains.

What is the source for the choices in "contact selector"? If it pulls
entries from a manually entered list (or even an existing SharePoint
list), you can make it so that the display name is friendly and
familiar while the bound value is the e-mail address.
