Could Not load an Object-Outlook Form

We have a user who was previously on Outlook 2000-they
have not upgraded to Office 2003, thus Outlook 2003.
Everytime she opens a Contact record she gets a Microsoft
Forms Dialog box with the error "Could not load an object
because it is not available on this machine". If she
click 'Ok' on that dialog box, a 'Just-In-Time Debugging'
dialog box appears asking her if she wants to debug using
the selected debugger'.

These errors did not occur and do not occur under Outlook
2000. I would appreciate any help you can give us.

Where is the form published? Does it use any ActiveX controls other than the
standard text box, combo box, etc. What folder holds the items that use the
form?
