Create a List Box on a Form

I need to create a list box to be used on a new form that
I have created. The Field that I am using is Profession -
and I want to add the following on a drop-down list:

Corporate/Securities
Employee Benefit/Tax
Energy
Environmental
Healthcare
Immigration
Intellectual Property
Labor/Employment
Litigation
Property/Probate

Can someone help? Thanks.

That worked great! Thanks much. Now, the only other
thing is this ... I want to allow the user to have
multiple items selected and the multi-line function is
grayed out.
