create a "search" form in Access (parent/child tables)?

Years ago I copied from an Access template (maybe household inventory?)--I
believe Access 2000, maybe earlier version . . . a form used solely to
search. The left side listed the fields to search (you clicked on the
selection button), and on the right side was a combo(?) box--when you clicked
on the different fields on the left, the combo box turned into a drop down
list that you could select from. The form was used to search 2 tables with a
parent/child relationship. I want to copy that form into my database &
change the table/field names in the code (what I did before) -- but I can't
find that search form anymore (I use Access 2003) now -- any suggestions on
how I can create, or where I can copy from, a form to search and retrieve a
specific record?
