create an editable template from a scanned form to be sent as a TI

My parent company requires a form to be filled out and sent to them via Email
as a "TIF" file with scanned attachments. It is a lenghthy form with lines.
I've scanned it and used "Paint" with text boxes and fill, but, their
scanners doesn't accept the mess the text boxes make of their lines. Is there
a way to create a editable template or form to be used with Word or Exel,
that can be converted to a "TIF" as a cover sheet for a multiple page
"mailing"?

To create electronic forms from scanned paper forms, you need special OCR
software such as OmniForm, which is now owned by Nuance.
