Create email using custom email form from Public Folder

Requirement: Create and Send email via CDO (or MAPI) by selecting a
published form in the Public Folders\All Public Folders\Forms

The idea is to simplify the publishing of forms to all users by using a
"public" form. The question is how to reference the public form
programatically (VB), create the email and send to internal recipients.

I have manually (via Outlook) selected the form and sent it to internal
recipients and it works fine.

Environment: Exchange 5.5
Visual Basic 6 SP5

Any advice would be appreciated.
