create labels with return address & shipping address

I would like to prepare shipping labels to show the company return mailing
address and to the company that it is being shipped to. Like an
envelope...Label size is 3X4

download free templates from Avery.com and design them at will via word....

even if you do not intend to use Avery labels, find one of their templates
that has
the same size as your actual label....
