Create User form

I'd like to create a form to e-mail to users so that when they type in the
spaces provided, the text behind their input does not move.

What you are talking about is what Word calls an "online form" _not_ a
UserForm. (A UserForm is a custom dialog box which requires visual basic for
applications programming. An online form is a type of Word document.) Check
