Creating a checkout form

I have a dbase for my engineering company's old jobs. I want to create a
computerized checkout system for this information to be used by company
employees. What I envision is a form that can pull job information that
already exists in a table in the dbase, let the user add their initials and
the date, and choose what part of the job info they have checked out
(different departments, and files or drawings), then have all this
information saved into a different table. Can this be done? If so, how?
