First of all, and most important, you must analyse the
requirements. What used to be called system analysis.
You need to have a good picture of what you want to achieve
and then you flesh that out with detail of fields and
tables, relations, reports, screen forms, backup strategy
and so on.
Sometimes the best way to start is to sit down and try to
list all the pieces of information you think you need to
record and see how things fall into place, doing normalising
almost on the fly.
Once that's done, you start coding.
The process is iterative, requirements change, analysis
shows other ways of doing things.
Of course, you could just dive in and start hacking away at
code. It might work.
