creating a postcard

I am wanting to create a postcard with printing on both sides. Front side
with our name & address on both the mail to: and the return addess: and on
the back side a short survey so that the customer can simply complete the
survey and drop it in the mail. I dont see a template that will allow me to
do that. Any help or suggestions?

I'm not sure what sort of help you want. The only difference between
creating what you want and creating a 2 page document is that you need to
set up the page size to postcard.

Then type your name and address on Page 1 and the questions on Page 2.
Print. Personally, I'd probably select the built-in Postcard template that
has 4 per page and write the addresses on page 1 and the questionnaires on
page 2. Then print onto standard A4/Letter card in duplex to create 4 cards
at a time.
