Creating a simple form with lines that can be typed upon.

I am using MS Word 2000. I am trying to create a simple form into which I
can insert lines that will remain static when they are typed upon. I am not
trying to create a web form. I thought that form fields would be the answer
but cannot understand how to use them.

Best bet are to put those formfields in table cells that have bottom
