Creating a "Submit Form" - HELP !!!!!!!!!!!!!!!!!!!!!!!!!!!!

Hello,

I am trying to create a submit form using Word. How do I get the Submit
button to email the full form to me ? I have three text fields within the
form. However, I do not know how to make the Submit button when hit send
the full text to me by email.

This way my email address stays hidden and I get the exact information I
require.
