Creating forms that allow "fill-in" areas, but keeps other text f.

Hello,
I've followed some tuts that explain how to make a form with "fill in"
areas. I've got the form & it looks exactly as it should, but the other text
moves once I try to fill in the areas. For example: Name ____________ Date
_________. Once I fill in the name section the date section moves as well.
NOT TO MENTION the underline part is a nightmare to figure out as well.
Please help me.

If you are typing into the form, what's the underline for? The PC knows
where to put the information!
To prevent the formatting from changing, put the form fields in table cells
of fixed size. You can use a bottom border for the cell if you must have the
underline.
