Creating forms

Hi there
Somone in this newsgroup once gave me link to an article
about good practice in designing forms that can be either
printed out and completed, or completed on-screen in Word.
I cannot find this link and wondered if anyone can
provide it or similar.
I have looked through the pages at http://word.mvps.org
but cannot see anything that applies...
