Ask Avery - it is not a part of Microsoft Office.

The 5150 label is a preprinted label with a space for the employee's name.
That one could be created from scratch with a little time. There already is
a template for that label (in Word 2007 - not sure about earlier versions
but maybe).

Hi Joey,

In Word 2007, using
Mailings=>Labels=>Options
and choosing Avery US Letter 8366 is listed. Were you looking for an Avery 5160 address label? There isn't a 5150 listed on the
Avery website.

There is/was a 5150 label by Avery. It was a preprinted label - kind of like
"Hello, my name is..." labels.

Hi JoAnn,

You're right, and I even have packs of them <g> (Employee peel & stick) , but it came up as 'not found' on the Avery site, by either
number or label description, althoug they're still listed on 3rd party sites.

Hi Joey,

The 5150 label dimensions are 2"h x 3 1/4"w with a page size of
4"x 6" (mini label pack). The 5444 would probably work although you may need to tweak it a bit to not print on the color parts or
use the dimensions to create a new label in Mailings=>Labels=>Options=>New Label
