custom BCM form has severe problems

I created a custom form, as per Pat's blog. I cleared the form cache several
times after. I followed full instructions on microsoft link posted here,

however I still cannot get a clean form. I have renamed
extend.dat,outcmd.dat,frmcache.dat as suggested, and created new profile
also.

Two problems:
1 When I try to enter new contact or open an existing one, I get the message
that "BCM has encountered a problem and needs to close. We are sorry...etc."
When I 'X' that window, my custom form appears, and BCM has not actually
closed.

2 However form has all of my email showing in the history, whether for new
contact or existing, and I have not linked any email yet. I've gone back and
cleared the cache again and again..... deleted the folders

After further reading here....I found the error report on this problem and
have cut and pasted here, hopefully someone can help.
