Custom design template issues

Running windows 2000 and office xp . . .

I've just set up a custom design template and it's not
working correctly. Each time I apply my template to an
existing or blank presentation an extra text box appears
on the title slide and the slide numbers don't appear on
the subsequent slides.

The mysterious text box which appears says 'click to add
subtitle' and is outlined. It is not part of the master
slide layout when I view the new/updated presentation.
The slide numbers disappear from the master slide layout
when I view the new presentation too.

I started my template with a blank presentation and added
my own pictures. Using the master layout feature, I added
a title, text, and slide numbers to my slide. Then on the
title slide master, I used the same feature to add only a
title box.

Any ideas for solving these two problems?
