Custom Icon on a Custom Toolbar?

Is it possible to display a custom icon for a command on a custom toolbar?

If so, where should the icon be located?

Well, I figured out how to do this from the customize menu. However, "Paste
Button Image" is grayed out. I copied a 16x16 bmp file to the clipboard
first.
