Custom Page Size For Printer By-pass feed creation

I am trying to create a custom page size so my client can select Letterhead as a page size, put their letterhead in the by-pass tray of the printer and have Word pull the page from that tray

In the past, we would setup a custom page size in Word, define the paper source as the manual tray, and we were done

This client has XP Pro and Office 2003. I have created the custom page called Letterhead in the Printers/Properties/Server Properties/Forms page, but it does not appear in the page size list when you go into Word/File/Pagesetup/Paper

To make things a little more complicated, the printer is a network printer on a peer-to-peer network. Each of the three systems print to a TCP/IP port rather than a local port. There are no other printers in the office

Word handles this in Page Setup by allowing you to choose the tray to print
from (this can be set for each document). If the letterhead is the same
paper size as one that is already defined, there is no reason for it to show
up. On the Paper or Paper Setup tab of Page Setup, choose Manual Feed for
the first page; if you want subsequent pages to be pulled from a different
tray, choose that for Other Pages.
