Custom Task Form Won't Register "completed"

I created a custom task form sometime back and added code to it so that it could be printed to a Word template. For over a year, the form worked fine. Recently the user reports that it cannot be marked completed (neither from the form itself nor from the list view of the Tasks folder). I'm flummoxed since the only code in it is there to allow the transfer of data into Word. Any suggestions?
