Customizing Contact button

I am trying to create a custom form where a user will be able to add a
contact from a shared contact folder in public folders. As I don't want to
create a completely new form I am customizing the contact form to use the
functionality I need from it.

Now, what I don't seem to find out is how to make the default folder for the
contact button in my form to be the shared contact folder in public folders.

Can anyone help me with this one please.
