customizing forms in bcm

i would like to customize the opportunity form (adding some fields) and was
wondering how to go about doing that. i might be missing a step. there is
no "customize form" on this option. i've looked in the "forms" option in
outlook but there seems to be a problem here. there's no opportunity
template to even download. any ideas?

also, still looking for a way to set up a report that displays the company
name.
