Date format in text box with VB coding

Help,
I have a text box with this info, but I want both the Start and End date to
be formatted with the mmm,yyy format.
Can someone help me with this code please.

="Performance Information " & [Enter a User Name] & "
for the period between " & [Enter a Start Date] & " and " & [Enter an End
Date] &" Format ([Enter a Start Date],"mmm,yyyy")

Format([Enter a Start Date], "mmm,yyyy") &
" and " & Format ([Enter an End Date],"mmm,yyyy")

(all on one line, of course)
