DB to Track Training Forms within an Office

Can anyone suggest a source for an existing sample database for tracking the
process of authorizing training forms within an organization? An optional,
secondary requirement is that each user's view should be limited to the
requests they submitted.

post a sample DB to this group or was the reply below meant as an
