I don't have one for OneNote but...
1. You could probably do it using the charting features of Word or
PowerPoint, then copy/paste into OneNote.
2. OneNote 2007's shapes and drawing tools make it easy enough to create
your own if you want to.
3. Visio is a powerful tool if you need extensive decision trees and org
charts.
