Delete confirm box not appearing

I have a user whose dialog box for confirming a record
deletion does not appear every time a record is deleted.
Occasionally the record is deleted without a prompt.

Any ideas for the lack of consistency?

Karen,
On the users PC, under the tools/options menu - In the Edit /Find tab
Check to see if Confirm Documents Deletions is checked.
