Disappearing Form After Profile Deleted

Hi

My profile in Outlook 2003 was corrupt so I deleted and created a new one. The problem is that when I've tried to configure one of my contact folders to use a specific custom form, the custom form seem to have vanished completely.

Even when I try to Design a Form, or Choose a Form, when I select my 'Custom Form' from Personal Library, it just opens as a regular Contact form

Any ideas? - Thanks for your tim

You might try clearing the forms cache: Choose Tools | Options | Other |
Advanced Options | Custom Forms, then click Manage Forms, then Clear Cache.
