do the bookfold horizontally

Hi,

I am trying to make a document(or template)in letter size
that can do the bookfold horizontally and have page
number on the right side. so each page is 4.25(height) X
5.5(wide) and can do the number in calendar fashion.
Does anyone know how to do it?
