You're asking in the wrong place. This newsgroup is for people
developing Microsoft Access databases; it is not a place to find
templates or documents.

In addition, you don't indicate where your wife is traveling FROM, nor
what laws or affidavits she might require. You should contact the
embassy of the relevant country directly; an affidavit that would work
for South Africa would probably not be useful in South Korea.
